Person-centered care means creating a plan that’s as unique as you are.

While we have four primary care offerings, the reality is much more complex. And much more personal. We recognize that no two residents are exactly the same, and we train our staff to find ways of providing the support that’s right for you.

Person-centered care takes many forms. Sometimes, it means working with you and your loved ones to evaluate your treatment options, thoroughly explaining the benefits and trade-offs of each. Other times, it’s as simple as a member of our team taking the time to connect with you on a deeper level.
